Failure Analysis
Products, components, and systems can fail for a variety of reasons, and failure is not always accompanied by a broken metal, plastic, wood, or other fractured surfaces.
The inability of a system, product, or component to reliably perform the function for which it was designed constitutes failure.
An accurate and thorough failure analysis to determine the origin and root cause frequently reveals more than one contributory cause played a role in the failure.
It is prudent to establish the proximate cause of failure and all contributory causes in order of importance or severity so that corrective action can prevent similar failures in the future.
Contributory Causes
Contributory causes include design deficiency, material selection, metallurgical fabrication, metallurgical processing, improper service usage, human performance, material defect, and damage from service or storage environment such as UV, ozone degradation, and corrosion damage.
